“大质控”理念下医疗质量管理体系的构建与实践探索

2021 年是“十四五”开局之年，国家先后印发了《关
于推动公立医院高质量发展的意见》《公立医院高质量
发展促进行动（2021—2025 年）》等相关文件，标志着
公立医院已进入了高质量发展的高速路 [1-2]。2021 年 10
月，国家卫生健康委办公厅《三级医院评审标准（2020
年版）实施细则》国卫办医发 [2021]19 号（以下简称
《2020 年版细则》）的印发，旨在指导医院加强自身建
设 和 日 常 管 理， 明 确 目 标 和 方 向， 促 进 医 院 实 现 高 质
量发展 [3-4]。《2020 年版细则》要求医院要有健全的医
疗质量管理体系，有明确的体系架构内容及运行机制，
并体现院、科两级责任制 [2]。但由于我国医院质量管理
的研究与实践起步较晚，存在质量管理流程不科学、队
伍能力不足、信息化程度低等问题 [5]。目前医院医疗质
量管理并没有一个成熟的管理运行模式，也缺乏一套完
整的医疗质量管理体系 [6]。而医疗质量管理体系完善与
否影响着医院质量与安全管理措施的具体实施，影响着
公立医院的高质量发展。医院结合“大质控”管理理念，
构建了闭环式的医疗质量管理体系，基于实践积极探索
体系高效运行管理措施，为新时期公立医院高质量发展
【摘要】 高质量发展是新时代发展的必然要求，也是深化医改
的重要内容。医疗质量与安全是医院持续发展的基石 , 医疗质
量管理体系完善与否影响着医院质量与安全管理措施的具体实
施，影响着公立医院的高质量发展。医院建立了“大质控”理
念下的医疗质量管理体系，涵盖了组织体系、标准体系、指标
体系、评价 / 反馈体系及综合质量目标考核体系，形成医院医
疗质量与安全闭环管理。基于实践，医院积极探索体系高效运
行新机制，促进了医院医疗服务能力和质量管理科学化、规范
化和标准化，实现了科学创新管理并取得实效，为引导和助力
医院高质量转型发展提供借鉴。

The Construction and Practice of Medical Quality Management
System Under the Concept of “Big Quality Control”

[Abstract] High-quality development is an inevitable requirement
of development in the new era and an important part of deepening
medical reform. Medical quality and safety is the cornerstone of
hospital sustainable development, whether the hospital medical
quality management system is perfect or not affects the specific
implementation of hospital quality and safety management, and
affects the high-quality development of public hospitals. The
hospital has established a hospital quality management system
under the concept of "big quality control", which covers the
organization system, standard system, index system, evaluation/
feedback system, and comprehensive quality target assessment
system, forming a closed-loop management of hospital medical
quality and safety management. Based on practice, the hospital
actively explored new mechanisms of high-quality and efficient
operation, promoted the scientific, standardized and standardized
hospital medical quality management, realized scientific innovation
management and achieved practical results, and provided reference
for guiding and assisting the high-quality transformation and
development of the hospital.
